Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant to support an in-house Skilled Nursing Facility. You will work under the supervision of a Physical Therapist to provide resident-focused therapy and assist with treatment planning.

Responsibilities include screenings, implementing interventions, documenting outcomes in NetHealth EMR, and coordinating schedules with the care team. Growth opportunities exist within a supportive, patient-centered environment.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying goals and treatment plans.
• Conduct group and individual treatments focused on strength, mobility, balance, and gait.
• Document compliant and medically necessary therapy notes in NetHealth EMR.
• Coordinate resident schedules with the Physical Therapist and care team.
• Educate residents, families, and staff on therapy approaches and safety.
