Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility to provide services under the supervision of a Physical Therapist.

You will conduct screenings, assist with evaluations and treatment plans, and help implement therapy programs focused on mobility, strength, and functional goals. The role requires an active PTA license and commitment to ongoing education. Full-time, with competitive pay and benefits.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• CMS expectations and NetHealth EMR documentation experience preferred.

Responsibilities

• Provide physical therapy services under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention or treatment.
• Assist the Physical Therapist in evaluations and treatment planning.
• Assist in establishing resident schedules with the Physical Therapist and team.
• Train residents, families and staff in use of therapy equipment.
• Monitor residents’ response to therapy and document progress per CMS in NetHealth EMR.
