Genesis HealthCare’s Powerback Rehabilitation To You program seeks a Physical Therapist Assistant to deliver outpatient rehabilitation in older adults within home environments. You’ll evaluate in-home settings or independent living facilities, supporting functional goals with supervision from a licensed PT.

The role emphasizes patient-centered care, flexibility, and collaboration with the rehab team to improve independence and quality of life for patients across various settings.

Qualifications

• Must have a valid license in the state(s) of practice.
• Must have a valid driver’s license or reliable transportation to and from patients’ residence.

Responsibilities

• Attend and participate in patient care and staff meetings as a representative of physical therapy.
• Adhere to department and patient treatment schedules; complete clinical and billing documentation.
• Collaborate with rehabilitation team to implement patient-specific programs.
• Assist the Physical Therapist during tests, evaluations, and complex procedures.
• Instruct patients and caregivers in therapy techniques under supervision.
• Maintain timely documentation for all treated patients.
• Ensure equipment is clean, maintained, and reported for repair as needed.
• Provide patient-centered care to promote independence and function.
