Job summary

The role emphasizes improving independence and functional outcomes for older adults, under the supervision of a Licensed Physical Therapist.

Qualifications

• Must have a valid license in the state of practice.
• Must have a valid driver's license or reliable transportation to/from patients' residence.

Responsibilities

• Attends and contributes to patient care and team meetings as a representative of physical therapy.
• Adheres to department and patient treatment schedules and completes timely documentation.
• Collaborates with rehabilitation team to implement individualized programs.
• Assists the Physical Therapist during tests, evaluations, and procedures.
• Instructs patients and caregivers in home exercise programs under supervision.
• Maintains timely documentation for all patients treated.
• Reports equipment needs and ensures proper maintenance and safety.
• Prioritizes patient service in a compassionate setting.
• Implements individualized treatment plans and provides therapies including therapeutic exercise, functional training, manual therapy, airway clearance, and wellness education.
