Job summary

TheCaseManager is seeking a dedicated Case Manager in New York to provide direct case management services for homeless individuals, addressing social, mental health, and medical needs to promote stability and empowerment.

You will work with a multidisciplinary team, completing intakes, developing psychosocial assessments, and coordinating housing and services to help clients live independently.

Qualifications

  • Bachelor's degree in Social Work, Psychology, Counseling or related field preferred.
  • Three to five years of case management experience with the homeless, mentally ill, substance abuse and/or low-income populations preferred.
  • Bilingual in English and Spanish strongly preferred.

Responsibilities

  • Completes intakes for new clients and provides an orientation to the program and shelter rules and regulations.
  • Reviews logbooks at the start of each shift to ensure that all clients follow-ups are conducted as required.
  • Participates as part of a team and regularly collaborates with multidisciplinary staff to monitor clients’ progress.
  • Develops a comprehensive psychosocial assessment to fully identify and address the client's needs.
  • Collaborates with internal staff to monitor clients’ medication compliance and address episodes of non-adherence.
  • Establishes an individualized intervention/treatment plan or strategy to promote identified goals.
  • Provides advocacy for individual clients around entitlements and services.
  • Provides case management, including individual counseling, crisis intervention, coordination of services and appropriate follow-up on referrals.

Education

Bachelor's degree in Social Work, Psychology, Counseling or related field
