UnitedHealth Group, in partnership with Amedisys, seeks a Physical Therapist Assistant to provide skilled home health care under a licensed Physical Therapist supervision. You will implement individualized treatment plans to improve mobility, strength, balance and independence while promoting safety at home.

You will be responsible for documenting progress, coordinating with patients, caregivers and the interdisciplinary team, and contributing to quality improvement activities.

Qualificações

• Graduate from an accredited PTA program as defined by CAPTE.
• Current license to practice as a PTA in the state you are assigned.
• Driver's license and access to reliable transportation.
• Current CPR certification.
• One year related PTA experience preferred for home health care.

Responsabilidades

• Provides physical therapy services under the supervision of a licensed PT in patients' homes.
• Observes and reports patient responses to treatment and any changes in condition.
• Documents supervisory activities and maintains patient records with care.
• Promotes wellness, safety and coordination with caregivers and the care team.
• Performs other duties as assigned.
