UnitedHealth Group's Amedisys home health division is seeking a Physical Therapist Assistant to work under a licensed physical therapist, delivering skilled care in patients' homes and implementing treatment plans to improve mobility, strength and independence.

You will document patient progress, collaborate with caregivers and an interdisciplinary care team, participate in care conferences and quality improvement activities, and help coordinate follow‑up care with other providers.

Competenze

• Current license to practice as a physical therapy assistant, specific to the state you're assigned to work in
• Graduate from an accredited physical therapy assistant educational program as set by CAPTE
• One year related physical therapy experience for PTAs entering home health care
• If less than one year of experience, approval from regional clinical leaders and mentoring/residency program required
• Current CPR certification
• Driver's license and access to reliable transportation

• Provides physical therapy services under the supervision of a licensed physical therapist
• Observes and reports patient responses to treatment
• Provides resources for future needs and promotes wellness
• Documents supervisory activities in the medical record
• Monitors and documents patient responses to therapy
• Participates in care conferences, in-services and quality improvement activities
• Maintains confidentiality of patient information
• Makes referrals to community resources and coordinates follow-up care
• Performs other duties as assigned
