Capital Health is seeking a Pharmacy Technician to obtain medication histories for new ED patients, documenting and communicating findings to ED pharmacists and staff. The role requires a CPhT certification, NJ registration within 6 months, and two years of experience.

You will work with the ED team to ensure accurate medication histories and participate in related quality initiatives. The position involves inpatient technician duties as needed, with a typical 40-hour week and comprehensive

Competenze

• High school diploma or equivalency; CPhT certification required.
• Two years of Pharmacy Technician experience required.
• NJ Board of Pharmacy registration or acquire the registration within 6 months of employment.
• Strong communication and interpersonal skills; attention to detail.
• Familiarity with EHR systems.

Mansioni

• Conducts detailed interviews with patients and/or caregivers to compile an accurate list of home medications including name, dose, route, frequency, and last dose taken.
• Prompt patient/family for OTC medications including herbals and vitamins.
• Contacts outpatient pharmacies and providers to verify medication regimens and investigate discrepancies.
• Obtains allergy history and severity for any medication or substance.
• Uses electronic databases and EMR to further medication history.
• Records medication and allergy information into the EHR for pharmacist/practitioner review.
• Precepts pharmacy technicians, students and residents regarding home medication interviews and documentation.
• Performs other duties and participates in Leap Frog data collection.
