Job summary

HomeFirst seeks a Case Manager for the Emergency Interim Housing (EIH) program in San Jose. The role supports unhoused adults, performs case management, and coordinates services to help residents transition to stable housing.

The site operates 24/7 with on-site staff and supportive services. Responsibilities include intake, individualized planning, community referrals, and collaboration with clinical and program teams to meet housing goals.

Qualifications

• Bachelor’s degree or 2 years case management experience may substitute.
• Experience building relationships with community partners outside the agency.
• Ability to manage multiple projects and meet deadlines.
• Strong written and verbal communication skills.

Responsibilities

• Welcome guests and provide direct service to participants.
• Maintain a caseload and develop individual plans with clients.
• Offer trauma-informed care and crisis support as needed.
• Maintain documentation in HMIS and ensure compliance.
• Collaborate with partners and participate in case conferences.
