Job summary

HomeFirst is seeking a Temporary Case Manager for the Overnight Warming Locations in San Jose. The role supports housing stability through Housing First and Harm Reduction approaches, enrolling participants, performing needs assessments, and coordinating benefits and referrals.

This position requires empathy, strong communication, and collaboration with partners. The role reports to the Program Manager and runs swing shifts as a temporary, non-exempt full-time benefit-eligible position through

Qualifications

• Minimum two years’ experience in social services; bachelor’s degree may waive experience requirement
• Lived experience or understanding of unhoused population needs
• Fluent in Microsoft Office Suite
• Strong written and oral communication skills
• High flexibility and prioritization ability
• Professional presentation to internal/external audiences
• Team player
• Bi-lingual a plus
• Able to lift up to 30 lbs and perform sedentary work

Responsibilities

• Provide case management including enrollment, needs assessments, budgeting, information and referrals
• Crisis de-escalation, safety planning, risk assessment, and mandated reporting
• Employment and benefits assistance; advocacy and partner collaboration
• Maintain open communication with supervisor and document progress
• Documentation in HMIS within 3 business days and maintain records
• Attend meetings/training and assist with office tasks and related duties
