HomeFirst is seeking a Case Manager for the EIH Mabury site in San Jose to support participants toward stable housing. You will manage a caseload, develop individual plans, and coordinate with clinicians, resident advocates, and community partners to ensure comprehensive services.

The role emphasizes trauma-informed care, strength-based engagement, and thorough HMIS documentation, with a focus on exits to permanent housing and collaboration across teams.

Qualifikationen

• Bachelor’s degree or two years of case management experience.
• Experience building relationships with community partners.
• Ability to manage multiple projects concurrently.
• Strong written and oral communication skills.
• Experience working with unhoused populations.
• On-site office-based work preferred.

Aufgaben

• Welcome guests and participants to the facility.
• Maintain a caseload with case management and direct service.
• Develop individual case management plans and support goal achievement.
• Provide community-based case management and referrals.
• Collaborate with community partners for housing support.
• Document in HMIS within 3 business days.
• Conduct participant orientations with providers and participants.
• Participate in Case Conferencing to support housing goals.
• Support trauma-informed care and CTI model.
• Maintain accurate records per grant and compliance standards.
