Job summary

Grady Health System is seeking a Clinical Care Coordinator (C3) to proactively identify barriers to patient flow, coordinate bed placement, and educate staff on patient status and resource use. You will serve as the primary liaison among the Emergency Department, Care Coordination Center, nursing, and support departments, partnering with physicians to ensure efficient patient throughput.

Qualifications

• Bachelor's degree required.
• 2+ years of RN experience.
• American Heart Association BLS required.
• Nurse Case Management experience preferred.

Responsibilities

• Act as liaison between ED, HUB, nursing and support departments.
• Educate medical staff on patient status, discharge planning, and resource utilization.
• Collaborate with physicians and care teams to ensure appropriate level of care.
• Communicate plan of care and discharge goals to patients and families.
