Job summary

Grady Health System, a major Atlanta hospital, is seeking a Clinical Care Coordinator (C3) who will proactively manage patient flow, guide care plans, and collaborate with multiple departments to ensure timely discharges and optimal patient outcomes.

This role requires a Bachelor’s degree and RN experience, with BLS certification; Nurse Case Management experience is preferred and contributes to leadership in patient throughput initiatives.

Qualifications

• Bachelor's degree required.
• 2+ years of RN experience required.
• American Heart Association BLS required.
• Nurse Case Management experience preferred.

Responsibilities

• Act as liaison between emergency and surgical services, nursing, and other teams to ensure proper bed assignment and patient flow.
• Identify and remove barriers to discharge and coordinate patient progression with physicians and care teams.
• Review admission orders to validate status and level of care; provide daily updates on patient progression and barriers.
• Coordinate care plans with physicians and MSW/CDI teams, ensuring appropriate sequencing of interventions.
• Communicate plan of care and discharge goals to patients and families, in collaboration with care team.
