Job summary

Grady Health System seeks a Clinical Care Coordinator (C3) to proactively identify barriers to patient flow and act as a liaison among departments and physicians. The C3 will educate medical staff on patient status, bed placement, payor requirements, and resource utilization to optimize throughput and LOS across the HUB.

The role emphasizes collaboration with nurses, service units, and external facilities to achieve safe, efficient, patient-centered outcomes within Grady Health System in Atlanta.

Qualifications

• Bachelor's Degree in a health related field.
• Three (3) or more years clinical experience.
• Critical Care, Emergency Department, or bed management experience preferred.
• Registered Nurse in the State of Georgia.
• BLS Certification.
• ACLS required for Critical Care and ECC units.

Responsibilities

• Proactively identifies and removes barriers to patient flow.
• Serves as a consultant and educator with medical staff regarding patient status and bed placement.
• Collaborates with physicians and service locations to ensure appropriate patient placement.
• Improves LOS performance through teamwork and process optimization.
