Job summary

Grady Memorial Hospital Corporation is seeking a Clinical Care Coordinator (C3) to proactively identify barriers to patient flow and drive bed placement and utilization of resources. The C3 will serve as a liaison across emergency, surgical services, nursing, and other teams to ensure appropriate care levels and timely discharge planning.

The role requires a Bachelor's degree and RN experience with BLS certification; Nurse Case Management experience is preferred.

Qualifications

• Bachelor's degree required.
• RN experience preferred, Nurse Case Management experience a plus.
• American Heart Association BLS required.

Responsibilities

• Act as primary liaison between units to optimize patient flow and discharge planning.
• Collaborate with physicians, nursing, MSW, CDI and other teams to coordinate level of care and resources.
• Review admissions and status for appropriate care level and timely discharge planning.
• Communicate plan of care and discharge goals to patients and families.
