Job summary

Grady Health System in Atlanta is seeking a Clinical Care Coordinator (C3) to proactively manage patient flow by advising medical staff on bed placement, status, and utilization of resources.

The role will act as the primary liaison between emergency and surgical services, nursing, behavioral health, finance, physicians, and utilization review to ensure appropriate bed is assigned and treatment plans are executed.

Qualifications

• RN experience required.
• American Heart Association BLS required.
• Nurse Case Management experience preferred.

Responsibilities

• Serve as primary liaison between emergency and surgical services, nursing, and other teams to ensure bed assignment and discharge plans.
• Collaborate with medical staff to optimize patient flow and LOS performance across departments.
• Review physician orders to validate status and level of care and perform e- Reviews as needed.
• Coordinate patient care plans with physicians and nursing, ensuring timely progression toward discharge and accessibility of status updates.
