Job summary

Grady Health System in Atlanta is hiring a Clinical Care Coordinator (C3) who proactively identifies barriers to patient flow and serves as a consultant and educator to medical staff on patient status, bed placement, payor requirements, and resource utilization.

The C3 acts as the primary liaison between emergency and surgical services, nursing, and other teams to ensure the right bed and appropriate care are in place.

Qualifications

• Bachelor degree required; RN licensure and active practice recommended.
• 2+ years of RN experience is required.
• American Heart Association BLS certification required.

Responsibilities

• Identify and remove barriers to patient flow across departments.
• Serve as liaison between emergency, surgical services, nursing, and other teams.
• Educate medical staff on patient status, bed placement, and payor requirements.
• Ensure accurate bed assignment and timely progression of care.
• Review admissions for level of care and status to support discharge planning.
• Collaborate with MSW, CDI, and clinical staff to progress care toward discharge.
• Communicate plans to patients and families about discharge goals.
• Support efforts to improve throughput and reduce length of stay.
