Job summary

Grady Health System in Atlanta seeks a Clinical Care Coordinator (C3) to proactively identify barriers to patient flow and coordinate bed assignment, care levels, and discharge planning across emergency, surgical services, nursing, and utilization review. The role acts as liaison to ensure timely transfers, accurate status updates, and patient-centered care.

Requires Bachelor's degree, 2+ years RN experience, BLS certification; Nurse Case Management preferred.

Qualifications

• Bachelor's degree required.
• 2+ years RN experience.
• American Heart Association BLS required.
• Nurse Case Management experience preferred.

Responsibilities

• Identify and remove barriers to patient flow and act as consultant for patient status, bed placement, payor requirements, and resource utilization.
• Serve as primary liaison between emergency and surgical services, nursing, behavioral health, finance, physicians, and utilization review to ensure appropriate bed and care level.
• Progress patient care toward discharge, coordinate plan with physicians and nursing and communicate with patients and families about plan of care and discharge goals.
