Job summary

Grady Health System in Atlanta is seeking a Clinical Care Coordinator (C3) to proactively identify barriers to patient flow and serve as a liaison among emergency, surgical services, nursing, and utilization review. Qualifications

• Bachelor's Degree required.
• 2+ years of RN experience.
• American Heart Association BLS required.
• Nurse Case Management experience preferred.

Responsibilities

• Act as a hub between units to facilitate patient progression and discharge planning.
• Collaborate with medical staff, MSW, CDI, and other teams to ensure accurate status in the system.
• Review admissions to validate status and level of care.
• Provide daily rounds or communications with physicians to discuss patient progression and discharge barriers.
• Coordinate care plans with physicians and nursing for appropriate sequencing of interventions.
• Communicate with patients and families about plan of care and discharge goals.
