Job summary

Grady Health System is seeking a Clinical Care Coordinator (C3) to proactively remove barriers to patient flow by serving as a consultant with medical staff on patient status, bed placement, payor requirements, and resource utilization. The C3 will liaise between the Emergency Department and the Care Coordination Center, nursing, and support teams to ensure accurate status and efficient patient placement.

Qualifications

• 2+ years of RN experience.
• American Heart Association BLS certification.
• Nurse Case Management experience preferred.

Responsibilities

• Serve as liaison between Emergency Department and Care Coordination Center to optimize patient flow.
• Collaborate with medical staff on patient status, bed placement, and resource utilization.
• Review admission orders to validate status and level of care.
• Round with physicians and communicate progression and discharge barriers.
• Coordinate care plans with physicians and nursing to ensure timely discharge.
• Inform patients and families about plan of care and discharge goals.
