Job summary

Ennoble Care is seeking a full-time Care Coordinator, Embedded in the Atlanta area (Sandy Springs vicinity). You will operate on-site within partnered long-term care facilities, coordinating care, documenting activities, and maintaining strong resident and family engagement.

The role emphasizes a hands-on, family-practice style of care with RPM integration and proactive coordination. Must have current clinical licensure and at least 1 year in long-term care.

Qualifications

• Legally authorized to work in the United States.
• Active clinical certification or licensure (MA/CMA/LPN/RN).
• Minimum 1 year of patient-facing experience in long-term care.
• Proficient with Microsoft 365 including Outlook, Word and Excel.
• Strong communication and teamwork abilities.
• On-site full-time role in Atlanta area (Mon–Fri, 8:30–5).
• Background check and drug screen required.
• Valid driver’s license with reliable transportation.

Responsibilities

• Coordinate care for long-term care residents using Ennoble Care’s EMR.
• Engage on-site with residents to support care plans and transitions.
• Collaborate with Ennoble Care providers in the long-term care setting.
• Act as liaison between providers, facility staff, residents, and families.
• Communicate patient needs and urgent concerns to the clinical team promptly.
• Document patient interactions and care coordination activities accurately.
• Follow workflows and escalation protocols.
• Contribute to team collaboration and facility partnerships.
