Job summary

Ennoble Care is seeking a full-time Embedded Care Coordinator to join our team in the Atlanta, GA area, focused on Decatur facilities. The role is on-site, requiring direct resident interaction, care coordination, and collaboration with providers and facility staff.

You will document activities, support care plans, and communicate patient needs to the clinical team while maintaining a strong presence within the facility. Strong organizational skills and professional communication are essential.

Qualifications

• Must be legally authorized to work in the United States.
• Minimum 1 year of patient-facing experience in long-term care or similar.
• Active, unrestricted clinical certification or Licensure (MA/CMA, LPN, RN).
• Proficient with Microsoft 365 including Outlook, Word, and Excel.

Responsibilities

• Coordinate care for long-term care residents using EMR.
• Engage on-site with residents to support care plans and transitions.
• Collaborate with Ennoble Care providers in the facility setting.
• Serve as liaison between providers, facility staff, residents, and families.
• Document patient interactions and care coordination activities.
