Ennoble Care is seeking a full-time Care Coordinator, Embedded to work at a Hamilton/Florence, NJ facility. You will manage individualized patient care plans, coordinate with clinicians and families, and ensure high-quality in-home/post-acute care through our EMR system.

The role emphasizes compassionate patient interaction, strong organizational skills, and proficiency with Outlook and EMR tools to maintain accurate documentation and seamless care delivery.

Qualifications

• Experience as Care Coordinator or similar in healthcare environment.
• Full-time, daytime hours at client’s senior living facility.
• Proficient computer use, including Outlook and EMR systems.
• Ability to pass a drug screen and background check.
• Strong communication and organizational skills.

Responsibilities

• Complete individualized patient care plans and perform care management using the EMR.
• Frequent contact with patients to provide care coordination and support.
• Document all client communications (verbal or written) accurately.
• Coordinate with Primary Care Clinician or designee about patient needs.
• Keep Team Supervisor informed of issues related to the care plan process.
• Collaborate with cross-organizational teams to meet patient needs.
• Become skilled at using technology including secure email and EMR systems.
• Adhere to documentation protocols and daily work logs.
