Job summary

Ennoble Care is seeking a full-time Care Coordinator, Embedded to work at a client’s senior living facility in Reading, PA. You will coordinate patient care, communicate with clinicians, and ensure high-quality services across care programs, including scheduling and support for caregivers and families.

The role emphasizes compassionate patient interactions, organized note-taking, and proficient use of EMR systems, with daytime hours Monday through Friday in Reading, PA.

Qualifications

• Must be comfortable speaking on the phone for long periods.
• Compassionate and empathetic toward patients with exceptional customer service.
• Able to take accurate notes and document tasks promptly.
• Able to multitask between patients and workstreams while remaining organized.

Responsibilities

• Complete individualized patient care plans and perform care management and care coordination services using Ennoble Care’s EMR.
• Frequent contact with patients to provide care coordination, support, and manage program compliance.
• Document all client communications accurately.
• Communicate with Primary Care Clinician regarding patient needs to ensure complete care plans.
• Keep Team Supervisor informed of issues related to the care plan process.
• Collaborate with cross-functional teams to meet patient needs and coordinate with families.
