Ennoble Care is seeking a full-time Embedded Care Coordinator in the Atlanta, GA area to provide on-site support at partnered long-term care facilities. You will coordinate care, engage with residents and families, and collaborate with facility staff while documenting activities and following established workflows.

The role emphasizes RPM and proactive care coordination in a family-practice style setting, with regular hours and on-site presence in the Decatur vicinity.

Qualifications

• Be legally authorized to work in the U.S.
• Minimum 1 year patient-facing care in long-term care or similar setting.
• Proactive, professional communication across multiple channels (Teams, email, phone, in person).

Responsibilities

• Coordinate care for long-term care residents using the EMR.
• Engage on-site with residents to support care plans and transitions.
• Collaborate with Ennoble Care providers in the long-term care setting.
• Act as liaison between providers, facility staff, residents and families.
• Document patient interactions and care coordination activities accurately.
