Job summary

Ennoble Care is seeking a full-time Care Coordinator, Embedded to work at a Westwood, NJ senior living facility. You will collaborate with clinicians, staff, and families to deliver high-quality, coordinated care and ensure clear communication across care teams.

Ideal candidates are compassionate, highly organized, and able to manage multiple care plans and caregivers. Proficiency with Outlook and MS Office, plus comfort with phone-based coordination, is required.

Qualifications

• Comfortable speaking on the phone for large portions of the day.
• Compassionate and empathetic towards patients.
• Able to multitask between different patients and workstreams while remaining organized.
• Proficient with computer software including Outlook and MS Office.

Responsibilities

• Create individualized patient care plans and perform care coordination using Ennoble Care’s EMR system.
• Frequent contact with patients to provide care coordination and support, and manage program compliance.
• Document all client communications accurately.
• Communicate with Primary Care Clinician regarding patient needs and life-threatening episodes to ensure complete care plans.
• Keep Team Supervisor informed of issues pertinent to the care plan process.
• Demonstrate ability to work with cross-organizational areas to meet patient needs.
• Become skilled at using technology including secure email, telephone system, EMR, etc.
• Adhere to documentation protocols and daily work logs.
• Provide excellent customer service as evidenced by positive feedback.
