Job summary

Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team in the Atlanta area, focusing on long-term care facilities in Sandy Springs. You will be on-site, building relationships with residents and families, coordinating care using our EMR, and collaborating with providers to ensure continuity of care.

The role requires licensure/certification, at least 1 year of patient-facing experience in long-term care, and strong communication and organizational skills.

Qualifications

• Legally authorized to work in the United States.
• Active, unrestricted clinical certification or licensure (MA/CMA, LPN, RN).
• Minimum 1 year of patient-facing experience in long-term care or similar clinical setting.
• Comfortable communicating proactively and professionally via Microsoft Teams, email, phone, and in person.
• Strong organizational and time-management skills with initiative.
• Ability to work full-time on-site in the Atlanta, Georgia area, Sandy Springs vicinity.

Responsibilities

• Coordinate care for long-term care residents using Ennoble Care’s EMR.
• Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions.
• Collaborate closely with Ennoble Care providers within the long-term care setting.
• Serve as a liaison between providers, facility staff, residents, and families.
• Communicate patient needs, changes in condition, and urgent concerns to the clinical team promptly.
• Accurately document patient interactions and care coordination activities.
• Follow established workflows, documentation standards, and escalation protocols.
• Contribute positively to team collaboration and facility partnerships.
