Job summary

Ennoble Care is seeking a Care Coordinator, Embedded to support on-site care within partnered long-term care facilities in the Leesburg area. You will join a close-knit clinical team, building trusted relationships with residents and families while coordinating care plans, follow-ups, and transitions in a patient-centered, family-practice style.

This on-site role emphasizes proactive communication, meticulous documentation, and teamwork with facility staff and Ennoble Care providers.

Qualifications

• Legally authorized to work in the United States.
• Active, unrestricted clinical certification or licensure (MA/CMA, LPN or RN).
• Minimum 1 year of patient-facing experience in long-term care or similar setting.
• Proficient with Microsoft 365 including Outlook, Word and Excel.
• Ability to work on-site Monday–Friday 8:00 a.m.–4:30 p.m. EST in the Leesburg/Reston area, no on-call.

Responsibilities

• Coordinate care for long-term care residents using the EMR system.
• Maintain regular on-site engagement to support care plans and transitions.
• Collaborate with Ennoble Care providers within the facility setting.
• Serve as liaison between providers, facility staff, residents and families.
• Document patient interactions and care coordination activities accurately.
• Follow established workflows and escalation protocols.
• Contribute to team collaboration and strong facility partnerships.
