Ennoble Care is seeking a Care Coordinator, Embedded in a Cincinnati, OH facility, to ensure high-quality care by coordinating with clinicians, staff, and patients. The role emphasizes compassionate, organized, and responsive care management within a home-based primary, palliative, and hospice care framework.

The position requires full-time work at the client facility, strong communication skills, and proficiency with Microsoft Office and EMR systems to support care plans and caregiver

Qualifications

• Ability to speak on the phone for long periods.
• Compassionate and empathetic toward patients with exceptional customer service.
• Proficient with Outlook and Microsoft Office.

Responsibilities

• Create and manage individualized patient care plans using Ennoble Care's EMR.
• Maintain frequent contact with patients to provide coordination and ensure program adherence.
• Document all client communications accurately (verbal or written).
• Coordinate with Primary Care Clinician or designee regarding emergent needs.
• Keep Team Supervisor informed of issues related to care plans.
