Job summary

Toledo Clinic is seeking a Records Clerk responsible for portaling and scanning paper documents into the EMR for upcoming patient appointments. You will review records to add comorbid conditions to patient problem lists and electronically process health information.

You will portal records from external systems, fax records to providers, and identify documents by name to ensure accurate electronic filing. Preferred candidates will have HS diploma, strong organizational and communication skills,

Qualifications

• Requires HS diploma or GED.
• Knowledge of comorbid conditions and ability to read medical charts.
• Ability to identify medical record documentation by name for electronic filing.
• Familiarity with HIPAA rules for releasing information.

Responsibilities

• Portaling and scanning paper documents into the EMR for upcoming patient appointments.
• Review patient records to add comorbid conditions to the patient problem list.
• Electronically process patient health information.
• Portal patient records from outside health systems.
• Fax records to physician offices or hospitals.
• Identify patient medical record documents by name.
• Handle requests for release of patient information per HIPAA rules.
• Other duties as assigned.
