Job summary

Capital Health is seeking a Pharmacy Technician to obtain medication histories for ED patients, document in the EHR, and support pharmacist and physician teams. The role involves direct patient/family interviews, collaboration with pharmacies, and potential inpatient technician duties.

Requirements include a high school diploma, CPhT certification, NJ registration (or within 6 months), and at least two years of technician experience.

Qualifications

• High school diploma or equivalent.
• CPhT certification required; NJ Board of Pharmacy registration or acquire registration within 6 months of employment.
• Two years Pharmacy Technician experience required; PTCB Medication History certification preferred.
• Strong communication and interpersonal skills; attention to detail; ability to prioritize and adapt to changing work volumes; basic computer skills and familiarity with EHR systems.

Responsibilities

• Conduct detailed interviews with patients and/or caregivers/family to compile an accurate list of current home medications including name, dose, route, frequency, indication for PRN, and last dose taken.
• Prompt patient/family members for additional medications including OTCs, herbals, vitamins, pain relievers, cold products, patches, lotions, eye drops, sprays, inhalers, etc.
• Contacts outpatient pharmacies and providers to verify medication regimens and investigate discrepancies.
• Obtains allergy history, type of reaction, and severity to medications or substances.
• Utilizes electronic databases and EMR to review medication history.
• Accurately records medication and allergy information into the EHR for pharmacist and practitioner review.
• Participates in performance improvement activities.
• Precepts pharmacy technicians, students, and residents regarding home medication interviews and documentation.
• Participates in Leap Frog data collection related to medication reconciliation.
• Function in all inpatient pharmacy technician tasks as needed.
• Perform other duties as assigned.
