Job summary

Elica Health Centers is seeking an ECM Supervisor to oversee the Enhanced Care Management team within the Social Care Department. You will ensure adherence to CalAIM ECM and MCP guidelines, mentor Care Managers, and coordinate care planning and client-centered outcomes.

You will also oversee documentation quality, support training initiatives, and partner with clinical consultants to implement patient-centered care plans across a diverse community.

Qualifications

• Bachelor's degree required or higher.
• Experience with care management and coordination within health care settings.
• Knowledge of HIPAA and medical documentation standards.
• Experience with care planning (SMART format) and SOAP notes.

Responsibilities

• Supervise ECM Care Managers and oversee day-to-day ECM operations.
• Ensure compliance with CalAIM ECM and MCP requirements.
• Coordinate training for new ECM Care Managers and ongoing staff development.
