Job summary

Optima Medical Management Group in California seeks a Site Manager to direct Serene Health's Enhanced Care Management Program (ECM). The Lead Care Managers coordinate physical health, behavioral health, and community-based long-term services for eligible beneficiaries.

You will lead a multidisciplinary team, liaise with health plans and external resources, develop and monitor Health Action Plans, oversee ECM protocols and documentation in the EHR, and drive compliant, high‑quality care across

Qualifications

• Bachelor's degree from accredited college or university.
• Minimum of 7 years of experience in a healthcare setting.
• Experience managing multidisciplinary care teams.
• Experience working with homeless, criminal justice, or other underserved populations.

Responsibilities

• Lead a multidisciplinary team to coordinate care for high-need populations.
• Liaise with Serene Health departments and external resources to coordinate care.
• Ensure Health Action Plan creation for ECM patients.
• Develop and implement care coordination protocols and policies.
• Manage ECM staff meetings and ensure program compliance.
• Oversee accurate ECM data documentation in the EHR.
• Travel to different sites to conduct training sessions if needed.
