Job summary

American TrueCare seeks a Site Manager to direct and coordinate its Enhanced Care Management Program (ECM). The role oversees multiple sites across California, leading a multidisciplinary team to ensure high-quality, coordinated care for high-need beneficiaries.

You will develop goals, establish staffing, manage documentation in the EHR, report on data, and ensure compliance with ECM rules while partnering with Health Plan stakeholders and community resources.

Qualifications

• Bachelor’s degree required.
• Minimum 7 years healthcare experience.
• Experience managing multidisciplinary care teams.
• Experience working with homeless, criminal justice, or other underserved populations.

Responsibilities

• Lead a multidisciplinary care team to coordinate care for high-need patients.
• Coordinate with internal departments and external resources to ensure care continuity.
• Develop and implement ECM protocols and staff training.
• Ensure Health Action Plans are created and followed for ECM patients.
• Maintain accurate documentation in the EHR and ensure compliance.
• Travel to training locations as required.
