Job summary

Elica Health Centers is seeking an ECM Supervisor to oversee the Enhanced Care Management team within the Social Care Department. You will ensure program compliance, coordinate training, and guide staff to deliver patient-centered care across our community programs.

The role emphasizes supervision, policy interpretation, and collaboration with the ECM Clinical Consultant to implement patient-centered care plans and achieve program goals.

Qualifications

• Bachelor's degree required.
• Minimum three years of Care Management experience.
• Proficiency with EPIC and care planning.
• Strong computer and Microsoft Office skills.
• Experience supervising or leading a team is required.

Responsibilities

• Supervise ECM Care Managers and oversee day-to-day operations.
• Ensure compliance with ECM and MCP requirements across the program.
• Coordinate training for new ECM Care Managers and ongoing staff development.
• Explain program requirements and policies to staff and new hires.
• Monitor and improve service delivery and client outcomes.
