Job summary

Elica Health Centers is seeking an ECM Supervisor to lead the Enhanced Care Management team under CalAIM. You will oversee day-to-day operations, ensure compliance with MCPs, train new staff and coordinate care planning to connect clients with medical, behavioral health and community resources.

This on-site role in North Highlands, CA requires a Bachelor's degree and at least three years in care management, with strong EPIC, Excel and Google Workspace skills.

Qualifications

• Bachelor's degree required; three years of care management experience preferred.
• Proficiency with EPIC and Client Centered Care Planning.
• Strong Excel and Google Workspace skills; able to work in Windows environment.

Responsibilities

• Oversee ECM Care Managers and daily operations to meet program goals.
• Ensure compliance with CalAIM ECM and MCP requirements; monitor quality of assessments and care plans.
• Coordinate staff training and onboarding; report program outcomes to leadership.
