Job summary

Symba Center seeks an ECM Manager to lead the Enhanced Care Management program, ensuring alignment with DHCS rules and the Model of Care. You will supervise RN Care Managers, Behavioral Health Care Managers, and coordinators, driving data-driven improvements and seamless care transitions.

The role emphasizes strategic program development, billing oversight, reporting, and caseload management to deliver high-quality, person-centered care for Members.

Qualifications

• Bachelor’s degree in a related field; clinical or health services background preferred.
• Experience leading ECM or similar care-management programs; CalAIM knowledge is a plus.
• Strong understanding of DHCS regulations and care-model standards.

Responsibilities

• Oversee ECM program compliance with regulatory and Model of Care requirements.
• Provide direct supervision to multidisciplinary ECM team and case reviews.
• Monitor performance metrics, outcomes, and program budgeting.
