Job summary

Symba Center in Victorville, CA is seeking a Registered Nurse Care Manager to join the Enhanced Care Management (ECM) team. You will support high-risk Members with complex medical conditions, lead assessment efforts, and coordinate with BH CM, CC, and CHW.

You will perform medication reconciliations, oversee transitions of care, provide clinical guidance, and ensure documentation aligns with ECM program standards.

Qualifications

• Bachelor's degree in Nursing required.
• Registered Nurse (RN) with California license and BLS certification.
• Experience in care management or ECM programs is preferred.
• Proficient with electronic health records and care coordination workflows.
• Strong communication and teamwork skills.

Responsibilities

• Provide complex case management for ECM Members with high-risk conditions.
• Complete medication reconciliations in collaboration with PCP and pharmacy services.
• Coordinate transitions of care with hospitals and care transitions teams.
• Offer clinical education on chronic diseases, treatment options, and healthy living.
• Track assessments and ensure completion of CHA/Shared Care Plans.
