Job summary

Symba Center in Victorville, CA seeks an ECM Manager to lead the Enhanced Care Management program, ensure regulatory compliance, and drive operational excellence. This role oversees billing, data integrity, and performance metrics while guiding a multidisciplinary team of care managers, coordinators, and health workers to deliver high-quality, person-centered care to Members.

Qualifications

• Bachelor's degree in social work, public health, nursing, or related field preferred.
• Minimum 3 years of healthcare management or intensive case management experience required.
• Current clinical license (RN, LCSW, LMFT, or similar) highly preferred.
• Ability to work flexible hours, including weekends and holidays.

Responsibilities

• Ensure ECM program aligns with DHCS and Model of Care requirements.
• Oversee program billing, claims, and data integrity for regulatory compliance.
• Monitor performance metrics and clinical outcomes to meet targets.
• Provide supervision to multidisciplinary ECM Care Team.
• Collaborate with CalAIM Director on growth initiatives and process improvements.
• Maintain patient registries and ensure care plans, screenings, and assessments are documented.
