Job summary

Clinica Sierra Vista is seeking a Manager of Enhanced Care Management to lead the ECM team across Fresno and Kern County. You will supervise staff, ensure program compliance with DHCS and Medi‑Cal guidelines, manage budgets, and partner with leadership to achieve program goals.

You will collaborate with clinicians and external partners to optimize patient care and program expansion while maintaining high performance standards.

Qualifications

• Bachelor’s degree in Social Services, Healthcare Administration, Public Health, or related field required; Master’s degree preferred.
• At least 5 years in a managerial or supervisory role, preferably in Clinica Sierra Vista or Medi-Cal setting.
• Strong leadership, organizational, and communication skills.
• Ability to motivate and manage a multidisciplinary team across locations.
• Excellent problem-solving and decision-making capabilities.

Responsibilities

• Oversee ECM staff training and compliance with ECM program guidelines.
• Collaborate with supervisors for clinical performance management and supervise staff.
• Monitor budgets and ensure ECM programs operate within grant and Medi-Cal budgets.
• Represent Clinica Sierra Vista’s ECM program at external meetings.
• Track program performance metrics for DHCS requirements.
