Qualifications

• Licensed Physical Therapist Assistant (IDFPR) in Illinois.
• Basic Life Support (BLS) within 30 days - American Heart Association.
• Board Eligible or must complete board exam when available.

Responsibilities

• Educates patients, family, caregivers, students and other health care providers using effective teaching methods.
• Follows the therapist's plan of care and adjusts treatment based on patient response with physician communication and daily notes.
• Produces documentation in accordance with the Clinical Documentation Policy (CTSCP67).
• If applicable, completes annual education to comply with DNV Hip and Knee Replacement program requirements.
• Performs treatment interventions including therapeutic exercise, gait/balance training, ultrasound, electrical stimulation, aquatic therapy; daily EMR documentation; coordinate with PT; communicate progress to referring provider.
