Job summary

Carle Health seeks a licensed Physical Therapist to evaluate and treat patients of all ages, tailoring plans of care and monitoring outcomes. The role emphasizes education for patients and families, and collaboration with staff to ensure safe, effective therapy and documentation in EMR.

A sign-on bonus is available for this position. Qualifying candidates will hold a Physical Therapy degree, IL PT licensure, and BLS certification with continuing education in stroke care as required.

Qualifications

• Bachelor’s degree in Physical Therapy required.
• Licensed Physical Therapist in Illinois or eligible to practice in IL.
• BLS certified within 30 days of start date.
• Continuing education in acute stroke care as required.

Responsibilities

• Designs an appropriate plan of care with goals, treatment and discharge plan.
• Educates patients, families and caregivers using effective methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Performs examination and uses reliable assessment methods.
• Produces documentation per CTSCP67 and CT policies.
• Screens patients to determine need for further therapy services.
• Collaborates with support staff and monitors care quality.
• Completes annual education for program requirements if applicable.
