Job summary

Carle Health seeks a licensed Physical Therapist (PT) to evaluate and treat patients of all ages, adjusting plans of care based on patient response and evidence. The role includes education for patients and families, documentation in EMR, and collaboration with the care team.

Sign-on bonus available. The department focuses on orthopedic and neurologic conditions, with a strong emphasis on safety and quality of care within our integrated health system.

Qualifications

• Bachelor's degree in a relevant field required.
• Licensed Physical Therapist (PT) in Illinois.
• BLS certification within 30 days (AHA).
• Completion of required continuing education for acute stroke care.

Responsibilities

• Evaluate and treat patients with plan of care including goals, outcomes, and discharge plan.
• Educate patients, families, and caregivers using effective teaching methods.
• Perform examinations, determine PT diagnoses and outcomes of care.
• Document in therapy EMR and maintain accurate records.
• Coordinate with staff to assign appropriate therapy tasks and utilize support personnel responsibly.
• Communicate progress and recommendations with referring providers.
• Complete annual education for DNV Hip and Knee program if applicable.
