Qualifications

• Bachelor's Degree in Physical Therapy required.
• Licensed Physical Therapist (PT) with state licensure.
• BLS certification within 30 days of hire; must maintain.
• Must complete required continuing education for acute stroke care.

Responsibilities

• Evaluate and treat patients of all ages; modify care plan as needed.
• Design and implement an individualized plan of care with goals and discharge plan.
• Educate patients and families using effective teaching methods.
• Document findings and treatment progress in EMR per policy.
• Communicate with referring providers regarding patient progress and treatment recommendations.
• Assist in annual education to comply with program requirements.
