Carle Health is seeking a dedicated Licensed Physical Therapist to evaluate and treat patients of all ages, adjusting plans of care as needed. The role serves as a clinical resource while modeling professional service delivery and education for staff and students.

This position offers a sign-on bonus and requires current PT licensure with IDFPR, BLS within 30 days, and ongoing CE in acute stroke care. The department emphasizes evidence-based practice and patient-centered outcomes.

Qualifications

• Licensed Physical Therapist (PT) with active IDFPR credential.
• Basic Life Support (BLS) certified within 30 days (AHA).
• Complete required CE for acute stroke care as applicable.

Responsibilities

• Demonstrates patient transfers safely with assistance as needed.
• Treats patients with dignity, empathy and engagement.
• Designs/adjusts plan of care with goals, outcomes and discharge plan.
• Educates patients, families, and caregivers using effective methods.
• Evaluates findings to determine PT diagnoses and outcomes.
• Performs examinations with reliable methods and documents results.
• Documents in the EMR per policy CTSCP67.
• Screens patients to determine need for further therapy services.
• Supervises support personnel and gives feedback.
• Completes annual education for HIP/Joint Replacement requirements if applicable.
