Job summary

Carle Health is seeking a Licensed Physical Therapist to evaluate and treat patients of all ages, updating plans of care as needed. The role involves educating patients, families, and staff while coordinating discharge plans to optimize functional outcomes.

Responsibilities include performing examinations, designing care plans, and documenting progress in the EMR. A sign-on bonus is offered and adherence to education requirements is expected for program goals.

Qualifications

Licensed Physical Therapist (PT) with active licensure in the applicable state.
BLS certification within 30 days of hire per AHA standards.
Strong foundation in patient education and evidence-based practice.

Responsibilities

Perform patient transfers safely and assist as needed.
Develop and adjust individualized plans of care with goals and discharge planning.
Educate patients and families using effective teaching methods.
Evaluate findings to Support PT diagnoses and outcomes.
Document findings per policy and maintain accurate records.
Screen patients for therapy needs and determine service duration.
Collaborate with support personnel and provide feedback on care.
Complete required education for HIP and program requirements where applicable.
