Qualifications

• Must be Licensed Physical Therapist (PT) in Illinois.
• Attend and satisfactorily complete continuing education on acute stroke patient care.
• Sign-on bonus offered for this role.

Responsibilities

• Perform patient transfers with proper body mechanics to ensure safety.
• Educate patients and families using effective teaching methods.
• Design and adjust a plan of care with goals, treatment, outcomes, and discharge plan.
• Complete documentation per CTSCP67 policy and maintain EMR entries.
• Evaluate clinical findings to determine PT diagnoses and treatment outcomes.
• Collaborate with staff to support patient care and ensure quality.
• Comply with DNV Hip and Knee Replacement program requirements where applicable.
