Qualifications

• Educates patients and families using effective teaching methods.
• Designs and adjusts plan of care based on patient response and evidence.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Produces documentation according to policy and uses therapy EMR.
• Communicates progress and recommendations to referring providers.
