Job summary

Mirage Post Acute is seeking a Staff Physical Therapist to evaluate residents, develop individualized treatment plans, and improve mobility and function. You will supervise PT assistants, educate families, and coordinate with physicians and staff to ensure quality care.

The role requires state licensure as a Physical Therapist and strong documentation, communication, and infection-control skills. On-site work with interdisciplinary teams is expected.

Qualifications

• Bachelor's degree in Physical Therapy; Masters/Doctorate preferred.
• Licensed as a Physical Therapist in the state; CPR if required.
• Proficient with Net Health and Point Click Care systems.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility.
• Supervise PT assistants and students in resident care.
• Communicate progress with physicians, staff, residents and families.
• Provide in-services and training for facility staff as needed.
• Maintain accurate documentation per policies and regulations.
• Maintain safe environment and follow infection control protocols.
