Job summary

Presidential Post Acute is seeking a Staff Physical Therapist to evaluate residents, develop individualized treatment plans, and collaborate with families and physicians to restore mobility and independence.

The role requires a Bachelor's in Physical Therapy with state licensure; Master’s/Doctorate is preferred, and familiarity with Net Health/Point Click Care is a plus. You will supervise PT assistants and ensure accurate documentation.

Qualifications

• Licensed as a Physical Therapist in the state.
• Bachelor's degree in Physical Therapy; advanced degree preferred.
• Proficient in computer systems used for patient documentation (Net Health / Point Click Care).
• Continuing education requirements must be met and maintained.

Responsibilities

• Evaluate residents to determine PT needs, functional limits, and rehab potential.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise Physical Therapy Assistants in resident care and related activities.
• Communicate progress, goals, and discharge plans with physicians, nursing staff, residents, and families.
• Complete timely and compliant documentation per facility and payer requirements.
• Contribute to quality improvement initiatives and rehabilitation program development.
